Adams Outpatient Behavioral Health opened its doors in 2004, offering counseling services to residents throughout the northeast Indiana counties of Adams, Jay, Wells, and neighboring Van Wert and Mercer Ohio counties. Adams Outpatient Behavioral Health is located at 815 High Street, Suite C, Decatur. Respecting privacy and offering comfort to our clients is of utmost importance for our team of staff which includes a program director, licensed mental health and addictions counselors and licensed clinical social workers, as well as a professional office management staff.
The goal of Adams Memorial Hospital’s counseling offices is to provide professional and confidential services to our clients, helping them to live productive lives in the community. All services provided by Adams Outpatient Behavioral Health will follow the patient privacy policies of the hospital. Professional and personalized care will be given in a private atmosphere that respects your feelings and needs.
Adams Outpatient Behavioral Health offers counseling services for persons of all ages, including small children, teenagers, adults, and seniors. Treatment options include individual, couples, family, and group therapy, depending on the client’s specific needs. The therapy staff has many years of experience working with depression, anxiety, stress, anger management, substance abuse, attention-deficit disorders, relationship issues, trauma, eating disorders, child and adolescent issues, grief, and parenting troubles.

Tim graduated from Grace College with a B.S. in Counseling and then from Indiana University with a MSW. He has been working at Adams Memorial Hospital since 2005. Tim is a Licensed Clinical Social Worker (LCSW) and a Licensed Clinical Addictions Counselor (LCAC). Tim has worked with a variety of individuals from children, teens, adults and seniors. He sees individuals with a variety of issues including mental health and substance abuse issues. Areas of interest include depression, anxiety, grief/loss, alcohol/drug issues; anger management, and marriage/relationship issues. Tim is interested in seeing individuals, families and relationships grow, change, find fulfillment and wholeness. And is interested in meeting with individuals that are looking to make minor changes in their lives, individuals that need support as they go through difficult situations or individuals that are looking to make major life changes.
Emily holds a Master’s Degree from the University of Saint Francis in School Counseling and in Mental Health Counseling. Emily’s under-graduate work was completed through Indiana-Purdue Ft. Wayne campus in Elementary Education. She also received education from Purdue University and is a registered nurse. Currently, Emily works with clients in North Adams Community Schools and Behavioral Health Outpatient office. Her well rounded work experience has given her a unique perspective with a depth of knowledge in health care and learning. A passionate area of practice for Emily is working with children, adolescents and their families. Emily’s clinical interest is in all areas of mental health. However, she excels in areas dealing with school issues, parenting, blended families, anxiety, depression, grief, attention deficit, and spectrum disorders. In her spare time Emily is working towards certification in Equine Assisted Psychotherapy.
Dr. Ishak is Board Certified in Psychiatry, and works with adolescents, adults, and seniors. He graduated from Ain Shams University in Cairo, Egypt, and completed his residency at Finch University in Chicago, IL. Dr. Ishak is the Psychiatrist for Adams Memorial Hospital’s Behavioral Health Department. Dr. Ishak has been with Adams Memorial Hospital for 10 years.
Adams County native, Leah received her Bachelor's degree in Psychology from Indiana State University and completed her Masters of Social Work (MSW) from Indiana University School of Social Work in Indianapolis, In. Leah completed internships at Vigo County Homes for Children, Lutheran Child and Family Services/Lutherwood, and Adams Memorial Hospital Behavioral Health Unit. Leah joined the Adams Memorial Hospital Behavioral Health Department in 2012 working in the inpatient unit as the unit therapist and has now transitioned to working as an outpatient therapist. Leah has experience in a wide array of issues including: substance abuse/dependence issues, depression, anxiety, relationship issues, communication issues, and self-esteem issues with all ages in both groups and individual sessions. Leah has a pure desire to help others make changes in their lives and looks forward to helping others explore and make desired changes in their lives. Leah is a licensed social worker and is currently working on obtaining her clinical licensure (LCSW). Leah currently offers counseling services at both our Decatur and Berne locations. After graduating from Ball State University with a Masters in Counseling Psychology, Carrie joined Adams Memorial Hospital in 2003 and works as a Licensed Mental Health Counselor. Currently, Carrie works as an out-patient counselor and has facilitated psychotherapy process groups at both the in-patient and out-patient levels of care. A passionate area of practice for Carrie is helping women work through struggles with eating disorders, self-esteem, body image, and relationship issues. Carrie has spent time in the local school system working with adolescents who have family life challenges, difficulty adjusting to change, or general mental health or behavioral problems. She takes a strengths based approach with her clients, works to emphasize their assets, and offers support and education focused on best clinical practices, including Dialectical Behavioral therapy skills, solution focused, and Cognitive Behavioral Therapy. Other areas of experience include depression, anxiety, grief/loss, domestic violence, and PTSD.
Rick Cain received his post graduate degree with a Psychiatric Nurse Practitionerer designation from IUPUI in Indianapolis, IN with Specialization in psycho-pharmacology, diagnostics and family, adult, child/adolescent therapy. He received his Master's of Science degree in Nursing Education as well as his Master's of Science in Management from Indiana Wesleyan University in Marion, IN. He received his undergraduate nursing degree from the University of St. Francis as well undergraduate degrees in General studies and Psychology from IPFW. He is a Board Certified Family Psychiatric Mental Health Nurse Practitioner and passed the board certification exam with the American Nurses Credentialing Center. Areas of specialty include: diagnostics including psychiatric illness and chemical dependency, psychopharmacological therapies including medication management, review and education, individual therapy, and mentorship, consultation and preceptorship for students and organizations.
