Patient Care Services

Coping with an illness and hospitalization can be a difficult and confusing time. During a hospital stay, decisions and sorting out information can become overwhelming. The Patient Care Services of Adams Memorial Hospital & the Extended Care Unit Social Services assists patients/residents and families with the concerns and challenges of the patient’s/resident’s illness and hospitalization.
The PCS staff is dedicated to providing individual care to our patients/residents and their families. We interact as part of the professional hospital team to improve the quality of life of each patient.


Medical Social Services are available to help you and your family. Anyone requiring assistance or information can contact the Patient Care Services Department at the following:

Hours: Monday–Friday 8:00 AM to 4:30 PM


Patient Care Services work as discharge planners to help access and coordinate additional health services. In collaboration with the healthcare team, a discharge plan is developed to manage health needs when the patient/resident leaves the hospital. During the hospitalization, options discussed may include:

- Hospice
- Nursing Home
- Assisted Living
- Durable Medical Equipment
- Emergency Response System
- Homebound Meals
- Advance Directives
- Mental Health Services
- Support Groups


Patient Care Services recognizes adjusting to illness and understanding medical care can be a difficult process. Patient Care Services helps patients/residents and families cope with emotional problems related to the illness. Our staff is trained to help with the following needs:
