Nick Lazoff, M.D.

Dr. Lazoff is a Fort Wayne native. He enjoys golf, fishing, IU sports and being involved with his children’s sports and academics.

Dr. Lazoff enjoys providing and caring for urgent care needs. He strives to create an environment of expedient and personalized care for his patients while respecting and facilitating the continuity of their treatment with their primary care providers.

Dr. Lazoff served for many years as an Emergency Department physician at Adams Memorial Hospital and in Corporate Medical Services, where he provided occupational medicine to many area businesses and industries. In recent years, he has served as a physician at StatCare, the hospital’s urgent care clinic.
