Our staff provides compassionate care to elderly patients in a variety of settings. Providers and nurse practitioners provide inpatient, outpatient and nursing facility care. We treat patients with memory problems, incontinence, gait instability, fall risks and chronic illness as well as those in need of extended care.
We offer the best health care available:
- Nurse practitioners available every day to address any health care needs
- Coordination with home care agencies and rehabilitation programs
- Medication management
- Therapy coordination
What We Treat
- Sensory impairment
- Pressure sores
- Drug-induced illness and iatrogenesis
Extended care is for patients residing in subacute care and nursing facilities served by our team. Adams Health Network nurse practitioners visit patients and provide ongoing medical care as needed. We focus on patients needing rehabilitation after hospitalization and those needing long-term care. We can assist with the coordination of services when our affiliated facilities are used.
