The days following a hospital stay are critical to patients’ long-term success. Mobile integrated health is a system designed to ensure that individuals who were recently discharged from Adams Hospital receive thorough follow-up care by members of our EMS team.

Mobile Integrated Health

Mobile Integrated Health leverages frontline responders – in this case members of Adams Hospital’s emergency medical services team – to coordinate follow-up home visits for hospital patients who are recently discharged. The goal of this service is to help patients transition safely from the hospital to their homes. Our Mobile Integrated Health team bridges the gap between a hospital visit and follow-up care from primary care providers, increasing the likelihood of a smooth recovery. Visits typically occur within 48 hours of discharge.

Prior to discharge, Adams Hospital’s inpatient team will help patients schedule an in-home visit with one of our Mobile Integrated Health paramedics. In most cases, we recommend that Mobile Integrated Health patients have a loved one or other support person present for the in-home visit.

Adams Hospital’s EMS team are the ideal professionals to fulfill these duties; they’re trained to respond to a wide range of medical emergencies and to quickly recognize health risks and potential issues.

What to expect during a Mobile Integrated Health visit

  • A member of our Mobile Integrated Health team will conduct a full set of vital signs, assessing blood pressure, heart rate, oxygen level and other important measures.
  • Thorough review of discharge paperwork, treatment plans and medication.
  • Confirm and coordinate a transitional care visit with your Adams Medical Group office.
  • Communicate concerns or questions to your primary care office to be addressed at the transitional care visit.
  • Assistance setting up or navigating through a patient’s MyChart account, the patient portal for access to electronic health records.

Kris Burkart, NREMT-P

Kris has 22 years of experience with Adams Memorial Hospital.  Kris focuses on delivering high-quality, comprehensive care with compassion to our community. Kris serves as our Manager of Adams County EMS and oversees our Mobile Integrated Health program.

PARAMEDIC
Hilarie Gaskill

Hilarie has worked in the EMS field for 18 years. She started as an EMT in 2005 and became a paramedic in 2009. She has worked at Adams County EMS for 14 years and also works part-time for Wells County as a paramedic. Hilarie is currently serving her second term as the Wells County coroner.

PARAMEDIC
Erin Purcell, Paramedic

Erin has been a paramedic for five years, with three years at Adams County EMS. She has a total of nine years of experience working in the emergency care setting between hospital and pre-hospital care. Erin became a paramedic because she enjoys caring for others and being part of their journey.
