Vaccine Clinic Information

Vaccines scheduled at Adams Memorial Hospital will be given at our Physician’s Office Building. The Adams Memorial Hospital vaccine clinic is located in the Adams Physician Office Building and is now in the same location as the COVID-19 testing clinic.

Hoosiers ages 5 and older can register for the COVID-19 vaccine by going to ourshot.in.gov, then searching for a nearby vaccine clinic and selecting an appointment time. Appointments may also be made by calling 211. Registration is preferred, but walk-ins are welcome. An adult must accompany a child; parental consent is required for minors. The appointment for the second vaccination will be scheduled after the patient receives the first dose. If a patient has already received the first dose of the vaccine, the option to schedule a COVID-19 vaccine appointment will not appear.

Scheduling COVID-19 Vaccine with Your Adams Primary Care Provider

Patients ages 18 and older with an Adams MyChart account now have the option to directly schedule their first COVID-19 vaccine appointment at any Adams Medical Group primary care location through MyChart or by calling 1-833-724-DOCS (3627) for assistance.

Testing Clinic Information

Free COVID-19 testing is available to all members of the public regardless of symptoms. Children as young as two can be tested with parental consent. Registration is preferred, but walk-ins are welcome.

To reduce your wait time, preregistration can be completed by calling 1-833-724-DOCS between 8:30 a.m. and 4 p.m. Preregistration can also be completed online at https://www.coronavirus.in.gov/2524.htm.
