Effective January 1, 2023, COVID vaccine and testing will be available in all Adams Medical Group Primary Care offices located throughout Adams County, by appointment only. To schedule an appointment for COVID vaccine or testing, please call 1-833-724-DOCS (3627).

Effective December 29, 2022 the Community COVID Testing and Vaccine Clinic located at the Adams Memorial Hospital Physician’s Office Building at 815 High Street in Decatur has been closed.

Scheduling COVID-19 Vaccine with Your Adams Primary Care Provider

Patients ages 18 and older with an Adams MyChart account now have the option to directly schedule their first COVID-19 vaccine appointment at any Adams Medical Group primary care location through MyChart or by calling 1-833-724-DOCS (3627) for assistance.
